SETTLEMENT CLASS MEMBER INFORMATION FORM

For the Class Members, payment will be sent automatically by First-Class U.S. Mail to the last known mailing address on file with the University Registrar.

Class Members also have the option, but are not required, to complete this Information Form to choose one or more of the following options: (a) provide an updated address for sending a check; (b) elect to receive the Settlement Benefit by Venmo or PayPal instead of a paper check; or (c) elect to have the Settlement Benefit applied to your Gannon student account.

* A valid email address must be provided to receive digital payment. If the email address you have on file changes or becomes invalid for any reason, it is your responsibility to provide accurate contact information to the Settlement Administrator to receive a digital payment.

* Your Notice ID is located on the upper right-hand corner on the Settlement Notice that was emailed or mailed to you. If you do not know your Notice ID, please contact the Settlement Administrator by emailing to info@GannonCovidSettlement.com, or calling toll-free 1-877-495-0978.
